- Once registered, LCU students can access the LCU Student Health Patient Portal using their LCU email address with the authenticator app. to upload their required immunization record and complete the required medical forms.
Required Immunizations and Medical Forms
Routine immunizations required for enrollment are:
- Quadrivalent Meningococcal (Meningitis) Vaccine — required by Texas law
- 3 available in the United States:
- Menactra: MenACWY-D
- Menveo: MenACWY-CRM
- MenQuadfi: MenACWY-TT
- must be within the 5 years prior to first class date of enrollment for all new and transfer students
- not required if age 22 or older
- get more info on the Meningitis immunization
- 3 available in the United States:
- Tetanus booster
- at least within past 10 years
- Tdap preferred – includes pertussis (whooping cough) booster
- MMR (Measles, Mumps and Rubella) — 2 shot series usually completed in childhood
The registered LCU student, using their LCU email address, can upload immunization records to the LCU Patient Portal.
Recommended Immunizations
- Hepatitis A
- Hepatitis B
- Varicella – if never had chickenpox
- Flu Vaccine Annually
- COVID-19 Annually
- An on-campus immunization clinic is typically available to the students each fall semester.
The three required Medical Forms are found on the LCU Patient Portal under the pencil/forms icon on the homepage:
- Health History Form annually
- Consent for Treatment annually
- Tuberculosis (TB) Screening Questionnaire
This form is ONLY required for students who answered YES to one or more of the TB screening questions on the TB Screening Questionnaire (found on the Student Portal).
This would include those who are from countries with a high prevalence of tuberculosis (TB). Please review this website for an up-to-date list of these countries.
If this applies to you, please print this form and take it to your primary health care provider or your local health department for further assessment. This should take place no sooner than 6 months prior to beginning attendance at LCU. The form should be filled out and signed/dated by a licensed health professional, and then uploaded on the LCU Student Health Patient Portal.
